Who can complete it?
Students or their representatives. It’s for anyone on a UEA programme - Undergraduate, Postgraduate Taught, Apprenticeships, and Postgraduate Research (PhDs, Professional Doctorates).

Details about your health, wellbeing, neurodivergence, or disability
Evidence of need (e.g. medical letters, diagnostic reports, or proof of previous adjustments — see our evidence requirements)
